Note: If a current Tri-merge credit report cannot be provided, the
client will need to complete the fee section (if providing your own
Tri-merge it should be no more than 30 days old)

1003 Application – 23 kb:
(This form is not needed on initial evaluation) Note: Although this
application is a Uniform Residential Loan Application we utilize
this for all loans, even Commercial up to $1,000,000.
Church Financing
Once these forms are completed, Fax them back to our office at
